In association with
Bridges to Excellence (BTE) and the Center for Health Transformation's
Healthy Georgia Diabetes and Obesity Project, a number of
large employers will implement the BTE Diabetes Care Link
(DCL) Program in Georgia in 2006. This program is about paying
more money to physicians who "do it right" and encouraging
diabetic patients to see those physicians to improve their
quality of life and avoid the long-term complications that
so often accompany this disease. In the process, physicians
are rewarded for providing high quality care, diabetics become
healthier and employers save money.
The Healthy Georgia Diabetes and Obesity Project
The Healthy Georgia
Diabetes and Obesity Project is being coordinated by the Center
for Health Transformation, a collaboration of private and
public sector leaders dedicated to the creation of a 21st
Century Intelligent Health System that we believe will save
lives, improve the quality of life, and save money. Creating
a system of better health at lower cost in Georgia is one
of the major undertakings of the Center; the Healthy Georgia
Diabetes and Obesity Project is one part of that effort.

Application
Fees and Reimbursement
- The survey tool
fee is $60 per medical office location.
- The physician
application fee is $300 for applicants associated with the
Georgia Bridges to Excellence program. If certified, the
application fee is refunded.
- Group rates
are available for practices of 7 or more physicians.
