Decide Which Program(s) To Participate In:
We sponsor programs for conditions such as asthma, cardiac care, diabetes, and more.
Your eligibility is based in part, on how many patients you treat with that condition. In addition, you should check to see whether your contracting health plans offer incentives for particular programs; many plans use our programs in their internal pay-for-performance initiatives.

Determine Which Performance Assessment Pathway Is Best For You:
BTE recognition is based on clinical data from your records, which you will submit for scoring. Submissions may be made directly to the BTE Portal or via any one of our EMR/Registry Systems partnerships or NCQA.

Apply For Recognition:
Depending on the pathway you have chosen, how you apply for recognition varies.

Recognition Maintenance:
All BTE recognitions are valid for 2 years, however if you would like to receive a higher recognition rating, you may do so after 90 days from your last submission.
Performance Assessment
In order to receive BTE recognition, eligible clinicians must pass a performance assessment program administered by Altarum or the NCQA.
Options by BTE Program
To obtain Recognition in BTE’s disease-specific Care Recognition programs, eligible clinicians and practices voluntarily submit medical record data demonstrating that they provide high levels of care for a specific condition. There are three paths available to clinicians for BTE Recognitions depending on the specific program.
BTE Care Recognitions

Direct Data Submission Portal: Altarum assesses compliance on all Bridges to Excellence (BTE) Recognition Programs submitted electronically or by direct data submission through the Bridges to Excellence (BTE) web portal.

Automated Performance Assessment System: Clinicians or practices connected to a BTE-approved data aggregator (e.g., EMR vendor, Patient Registry vendor, Health Information Exchange, or Health System) may have their data submitted on their behalf to Altarum for an automated evaluation of their care for a specific BTE Care Recognition program. Interested clinicians should contact their vendor or organization for more information.
